Summary

Introduction: Deep neck infections (DNIs) represent serious infections involving the fascial spaces of the neck, often of polymicrobial origin including both aerobic and anaerobic pathogens. Accurate microbiological diagnostics (encompassing specimen collection, transport, and culture) are essential for effective treatment and prevention of complications. Methods and material: A retrospective analysis of DNI cases at the Otorhinolaryngology clinic of the University Hospital in Pilsen (2015–2023) demonstrated a high prevalence of anaerobic bacteria and polymicrobial etiology. Results: The most commonly isolated organisms were Streptococci (35%), Fusobacterium spp., and Prevotella spp. Conclusion: Antibiotic resistance among pathogens causing DNIs is becoming an increasingly frequent issue. Streptococci show rising resistance to clindamycin, and a significant increase in resistance is also observed among anaerobic isolates of the genera Prevotella and Bacteroides. This highlights the need for targeted antibiotic therapy based on culture and susceptibility testing. A multidisciplinary approach, early diagnosis, and continuous monitoring of local resistance patterns are also essential for appropriate selection of empirical therapy.
