Aspirační punkce v léčběperitonzilárního abscesu I(Obecná část)

Incize je u nás dosud nejčastějším řešením peritonzilárního abscesu (PTA). V západníchzemích je v posledních letech při léčení PTA preferována aspirační punkce (AP) v kombinacis antibiotickou léčbou. AP je prováděna v místě největšího vyklenutí abscesu, v případě potřebyještě z dalších vpichů. Ve většině případů stačí pouze jedna punkce. AP může být opakována, nebodoplněna dle potřeby incizí či tonzilektomií. Výhodou AP je velmi dobrá tolerance pacienty,dostatečně vysoká účinnost (vyléčení v 80 - 95 %) a velmi nízké léčebné náklady. Pacienti léčeni APnemusí být hospitalizováni. Pro svou jednoduchost může být v případě potřeby prováděna i prak-tickými lékaři (západní země). První PTA není obvykle indikací k tonzilektomii. Recidivující PTAnebo PTA u pacientů s anamnesticky častými tonzilofaryngitidami jsou indikací k oboustrannétonzilektomii, nejvýhodnější je pak tonzilektomie za tepla.

Overview

Incision is in this country so far the most frequent approach in peritonsillar abscesses(PTA). In western countries in recent years in the treatment of PTA aspiration puncture (AP)combined with antibiotic treatment is preferred. AP is performed at the site of maximum bulgingof the abscess, if necessary, several punctures are made. In the majority of patients one puncture issufficient. AP can be repeated or supplemented, as required, by incision or tonsillectomy. Theadvantage of AP is very good tolerance by the patients, high effectiveness (cure in 80 - 95 %) andvery low costs of treatment. Patients treated by AP do not require hospital admission. Due to thesimplicity of the procedure it can, if necessary, be performed also by general practitioners (westerncountries). A first PTA is usually not an indication for tonsillectomy. Relapsing PTA or PTA inpatients with frequent tonsillopharyngitis in the case-history are indicated for bilateral tonsillec-tomy, hot tonsillectomy being best.
